All prospective patients begin by completing a brief screening form, which helps determine whether care at Pax Mentis Psychiatry is an appropriate fit for your needs and expectations before scheduling.
If it appears to be a good fit, you will be invited to schedule an appointment and complete additional intake forms prior to your visit. This allows appointment time to be used thoughtfully and efficiently.

Initial Psychiatric Evaluation

Extended Appointments

Consultation & Second Opinions
Pax Mentis Psychiatry offers focused consultation services for individuals or families who already have a primary psychiatrist, pediatrician, or other treating clinician and would like additional expert input.
